You have been asked by OISC to use Veri-fy ® to confirm your identity.
To do this you will need to:
- Complete online a Veri-fy® form and print it out
- Take the form together with the documents you have selected and an envelope to any of the 12500 Post Office® branches
- The Post Office® staff will verify your identity by completing the Veri-fy® form and sending the form off in the envelope.
- Your ID documents will be returned to you
- There will be a charge of £5.11 payable at the Post Office® for this service
To complete your Veri-fy® form you will need to have in front of you:
Your passport - if you have one
Your UK photo-driving licence (full or provisional) - if you have one
Two of the following, one of which MUST confirm your current address.
- Addressed Pay Slip
- Bank/Building Society Statement
- Benefit Statement e.g. Child Allowance
- Council tax statement
- Credit Card Statement
- Electricity Bill
- Gas Bill
- Insurance Certificate
- Mail Order Catalogue Statement
- Mobile Phone Statement
- Mortgage Statement
- NHS Card (UK)
- Store Card Statement
- Telephone Bill
- Television Licence
- UK Certificate of British Nationality
- UK Court Claim Form
- UK Entitlement Document From Central / Local Government / Government Agency / Local Authority
- Vehicle Registration Document
- Water Bill
- Work Permit/Visa (UK)
You will also need the following.
- Completed and signed CRB disclosure form
- Stamped and correctly addressed Envelope
You will also need a cheque or postal order for £26.00 made out to "Criminal Records Bureau".
